2026 Liability Waiver

2026 Liability Waiver and Release.

Required for every player and coach at the 2026 Christmas Classic, December 27–30, 2026, at FCP Sports, 33 Jet Drive NW, Fort Walton Beach, FL. Read the waiver, then sign below.

In consideration of being allowed to take part in the 2026 Holiday Hoops Classic (the "Event"), the undersigned agrees to the following on behalf of the team and its players, coaches and staff (the "Participants").

1. Assumption of Risk

Basketball is a physical contact sport. Taking part involves inherent risks, including sprains, fractures, concussions, other serious injury, illness and, in rare cases, death. These risks can result from contact with other players, equipment, playing surfaces and the actions of others. Each Participant is physically able to play. The undersigned understands these risks and voluntarily assumes all of them, known and unknown.

2. Release of Claims

To the fullest extent permitted by law, the undersigned releases, waives and agrees not to sue FCP Sports, the Holiday Hoops Classic, their owners, officers, staff, volunteers, officials and sponsors, and the venue and its owners and operators (the "Released Parties") for any claim, injury, loss or damage arising from the Event. This includes claims based on the ordinary negligence of the Released Parties.

3. Medical Treatment Authorization

If a Participant is injured or becomes ill, the undersigned authorizes Event staff to call emergency medical services and authorizes licensed medical providers to give the first aid and emergency treatment they consider necessary. Event staff will make reasonable efforts to reach a parent or guardian. The undersigned is responsible for the cost of any treatment.

4. Photo, Video and Livestream Consent

Every game is livestreamed. The undersigned permits FCP Sports and the Holiday Hoops Classic to photograph, record and livestream Participants during the Event, and to use those images, video, names and likenesses in broadcasts, websites, social media and promotional materials, without payment.

5. General Terms

This waiver is governed by Florida law. If any part is found unenforceable, the rest remains in effect. It binds the Participants and their heirs and assigns. The undersigned has read this waiver, understands it and signs it voluntarily.

Players under 18 must be signed for by a parent or legal guardian. Players 18 or older may sign for themselves.
